Complete the following information below each year to help determine eligibility for and the amount of 15-year catch-up allowed. The amount available under the 15-year catch-up provision is based on number of years of service and total contributions previously made. Each year your 403(b) deferral contribution is increased by the lesser of:
- $3,000
- $5,000 multiplied by the number of years of service less the total amount of your elective deferrals (including Roth deferrals and 15-year catch-up).
- $15,000 minus prior year 15-year catch-up contribution amounts (both pre-tax and Roth)

If both Age 50 catch-up and 15-year catch-up are allowed you must use 15-year catch-up first.
